fix: respect triggerPatterns config, allow all messages when empty
...
- Check all trigger patterns, not just first one
- When triggerPatterns is empty, intercept all messages
- When configured, only process trigger/mention/thread-reply messages
- Remove DM auto-intercept when triggers are set
